What care you should expect from your diabetes care team

Annual Review
Laboratory Tests and Investigations
- Blood glucose control : HbAIC
- Kidney function: urine and blood tests to check for protein.
- Blood fats: cholesterol and triglyceride.
- Other blood test may also be done if required.
Physical Examination
- Weight and body mass index
- Blood pressure
- Legs and feet examined to check skin, circulation and nerve supply.
- Eyes examined by 'Fundoscopy'. Also retinal photographs to record the
appearance at the back of the eyes.
- If using insulin, injection sites should be examined.
Lifestyle Issues
- Your general well-being and how you are coping.
- Your current treatment.
- Your diabetic control including home monitoring.
- Any problems you may be having.
It should include discussion about smoking, alcohol consumption, stress, sexual
problems, physical activity and healthy eating issues. You should feel free to
raise any or all of these issues with the Diabetes Care Team.
Diabetes Reviews
These are generally 3 or 6 monthly depending on need. But could be more
frequent if required eg. at diagnosis or when experiencing problems. If urgent
advice is needed you can speak to the Diabetes Nurse by phone on - (01562)
