We are a team of highly experienced District Nurses. We are employed by Wyre Forest Primary Care Trust and contracted to work for the Church Street Surgery. Our role is to work with other members of the Primary Health Care Team to deliver best, evidence based practice in the primary prevention and management of ill health for the housebound practice population. We believe that our team philosophy sums up very well what we are about; ‘We are a group of professionals dedicated to providing skilled, holistic nursing care to patients in their own homes. In collaboration with the Primary Health Care Team we strive to promote and maintain each individual’s health and independence whist respecting their uniqueness and right to privacy and dignity. Through ongoing education and training we are committed to the provision of safe, effective, high quality nursing practice.’ District Nurses have a Trust wide responsibility to be involved in Clinical Governance issues and Clinical Supervision in order to continually monitor and improve the quality and consistency of our service to patients across the Wyre Forest. We also have a clear educational role in the sharing of best practice with students, carers, patients, colleagues and other members of the Primary Health Care Team. We have responsibility for the human, financial and material resources at our disposal and to ensure their effective and efficient use in the delivery of quality patient care.
